Privacy Policy

One of our social responsibilities is to protect Personal Information and Right by appropriate information management and control. We will share Privacy Policy, listed below, with all doctors and staff in KOSHIKAI, and will consistently make all efforts to commit protection of such Personal Information and Right in daily operation.

1. Collection, Use, and Provision of Personal Information

We shall establish appropriate security standard and policy to manage Personal Information. We shall comply with these security standard and policy when we collect, use, or provide any Personal Information.

2. Safety and Security Measures of Personal Information

We shall establish safety and security measures to prevent the loss, destruction, falsification or leakage of Personal Information from illegal access and computer viruses. If the worst case does happen, we shall take prompt corrective action as well as preventive action against such case.

3. Correction, Discontinued Use, and Deletion of Personal Information

We shall receive request on disclosure, correction, discontinued use, and/or deletion of Personal Information from each patient. As soon as we confirm patient's identity, we shall take appropriate action to such request under our internal rule.

4. Compliance with Laws and Regulations of Personal Information

We shall comply with any laws, regulations, guidelines, and rules related to protection of Personal Information.

5. Training Program for Personal Information Protection

We will provide necessary training program and education for protection of Personal Information to our doctors and staff, and will improve our internal rules in order to maintain appropriate Personal Information Protection.

6. Provision and Disclosure of Treatment Information

We separately define provision and disclosure of Treatment Information under our internal rules.
